Transition Support
The Transitions Support Navigators, will support rakatahi when they prepare to exit Care by providing guidance, resources, and advocacy. We work closely with partner agencies to ensure rakatahi have wraparound support to become valued and contributing members of our hāpori.
Transitions Navigators empower rakatahi in realising and walking in their own mana motuhake.
We support rakatahi from 15 - 21 years who have spent at least three continuous months in care or your justice placements after age 14 years and 9 months.
How to Apply
To apply you must complete an application. We can give you an application pack either in person or send it by email or post. You choose.
When tamariki come into your care, your kaimahi will be there to give you on-going support. At the outset, you and your Kaimahi will develop a support plan to identify your needs as a kaitiaki and how we can best support you.
