Transition Support Service

The Transition Support Service is here to guide rangatahi as they move from state care or youth justice placements into adulthood. We provide trusted relationships, cultural identity support, and practical life skills to ensure they have the foundation to succeed independently.

Who We Support:

  • Ages 16-21 – Rangatahi who have spent at least three continuous months in care or youth justice placements after age 14 years and 9 months.
  • Independent & Connected Rangatahi – We support those living independently and those maintaining whānau ties.
