Coordinated Community Care

Te Kāika’s Community Connector service in Dunedin provides whānau with personalised, wrap-around support to access essential community health services, including immunisations, social services, and wellbeing support. Our kaimahi work alongside individuals and families to identify needs, remove barriers, and connect them with the right services—quickly and with care.

This service is designed for those who may struggle to access mainstream healthcare systems, offering a holistic, whānau-centred approach that supports long-term wellbeing.
